Can I use my health insurance?

Unfortunately I am an Out of Network provider and I do not bill insurance directly. For most major medical insurances I can provide you a super bill but you will be charged a Time of Service rate.

What About Medicare?

Elevation Sports Medicine is a private-pay wellness and sports medicine practice. Certain services may not be covered by Medicare or may not be submitted by this office. Coverage depends on medical necessity and Medicare rules. Please reach out before booking to ensure you are a good fit for this style of clinic and to go over any questions you may have regarding care and coverage.

What is a super Bill?

A super bill is a detailed receipt you get from a healthcare provider after a visit. It is not a bill asking you to pay (you have already paid). Instead it is paperwork you use to try and get reimbursed by your insurance.

How does a super Bill work?

1) You pay your doctor/therapist directly (often they are out of network).

2) They give you a super bill with all of the official codes and details.

3) You send that super bill to your insurance company.

4) Your insurance may send you some money back, depending on your plan.

How much is a visit?

This depends! Please see the Pricing tab for more information.

Do you accept HSA or FSA cards?

Yes! "Cash-Pay" simply means you are paying at the time of service and the clinic will not be submitting an insurance claim on your behalf. I can accept Cash, Check, Debit or Credit Card, and HSA/FSA.

Where is the clinic?

I am located ON THE SIDE of 2201 Portland Rd (the cross street is Hulet St).

Where do I park?

Please park on the street (Hulet St.) or in the gravel parking lot on Hulet St. There is parking available behind the building but not much, you can also park in the front of the building and walk around to the side- these are not parking spots assigned to me specifically.

Should I arrive early?

As a "one woman show" the schedule is designed for each patient to have their full treatment time plus time for administrative tasks (i.e. scheduling and payment). Please don't arrive more than 5 minutes early to your appointment to allow the patient ahead of you their full time.

THIS IS AN OPEN CONCEPT CLINIC- if you wouldn't want someone walking in on you, please don't walk in on someone else. There is a sign posted when a treatment is in session.
